Can these be used to heat a water bed mattress?

If you’re only looking to keep it from freezing I imagine it’d work fine. Otherwise it turns off way before a comfortable temperature.

Could this be used to warm batteries?

I imagine it could but I do not know what the max output power is. I wouldn't until you are sure of the pads max operating temp and check the battery specs to see it's heat tolerance. Essentially these are just heating pads with a built in on/off sensor. It would warm whatever it is stuck on.

Can you connect this directly to pos/neg battery terminals on a 200Ah 12v auxiliary battery?

That is the intention. However, highly recommended to add the switch pack. At least in an Airstream - which ducts heat to the tanks - you have the option of turning power to them completely off. Also, while your RV is idle with batteries still in place and nothing in your tanks, you can insure they're off and not draining the battery when outside temps go below 42 (when they automatically turn on).
