what are the monthly expense to run

To update Mike's answer for 2022. 7 watts per foot @ 30ft = 210watts. Divide by 1,000 = .21kW x 24 hrs = 5.04 kWh per day x cost of electricity in your area (this ranges from $0.2254 to $0.0937 depending on your state) so it could range anywhere from ~$0.47 to ~$1.14.

Can this be used on pex plastic pipe?

You can use it on plastic pipe if the pipe will always be filled with water, but don't wrap it - just run a straight line. See the Frost King website: http://www.frostking.com/wp-content/uploads/Pipe_heat_cable_trifold.pdf

is it thermostat controlled and if not, at what temperature will it be activated? there is existing pipe insulation can the cable go on top of it?

Frost King HC6A 6 Feet Automatic Electric Heat Cable Kits, Black Keep your pipes from freezing in the winter. With a built-in thermostat, the Frost King Electric Heat Cable Kit automatically keeps your water pipes from freezing. No more frozen pipes! For use on 2 in. metal and plastic water pipes. Keeps water flowing down to -40 degrees Fahrenheit. Plug has a power indicator light.

Is it safe to have the cable wrap the pipe inside a foam pipe insulator cover, or will it melt the foam insulator?

I ABSOLUTELY had it melt foam insulator before. Do NOT overlap it. I have pics of where it burned through foam pipe insulator because I sued contractor for almost catching my home on fire
