Q: How and why did you choose between the 15" and the 18"?

My mom is a small women (5'5" less than 100 pounds) so didn't need anything large. Also, I wanted to be sure it would fit in any chair and most surfaces.

Q: Can this be used in bed ?

Yes it can. I was stuck in a hospital bed for a week but was not allowed to lay flat so there was a lot of pressure on my already painful tailbone. I used this and it made all the difference in the world!

Q: would this support my head if i used it when getting a backrub?

I believe it would. You can inflate it as much as you want.
