Do these seal without a sealing machine? How are they put together?

Yes, these do come without a sealing machine. There is an adhesive. You peel off the back and stick to the blister. You can purchase the "weekly blister pack kit" which also contains a tray to set the pill blister in for easier loading of your pills.

The picture for the monthly x-large is the same as the picture for the weekly x-large. Is the monthly x-large labeled with dates instead of days?

It is the same product. The monthly is used for once a day and the weekly would be used for 4 times a day. You could also set up 2 weeks of pills if you use the twice a day format (AM/PM)
