is this attached on two or three hinges?

2 hinges. I have a 2 hinged door. I cannot remember if there was hardware included for a 3 hinged door. The kit included 2 screws that screw into the wall to help stabilize the rod.

How big a towel can hang on it ?

You can hang a regular bath towel, folded in half lengthwise, on it. I think if you tried the bath "sheet" sized towels, especially 4 of them, it might be too much weight & the towels may touch the floor. Regular size ones are fine though.

Do the arms swing on the hinges so that they can be pushed to the back of the door or pulled away from the wall for ease of access

Hi D. Yes, the arms rotate independently and can swivel on the hinge. That means they can move toward or away from either the wall or door. Hope that helps!

Are the hinge attachments adjustable or does my door have to have some kind of standard spacing between the hinges?

it will fit two or three hinge doors, the brackets are adjustable and the directions to put it together are good. You just need a tape measure, screw driver and hammer to remove hinge pins, they even enclose a flat nail that you use to help pop out the hinge pins.
