Q: Do these have an arch or are they flat? I need flat insoles to wear under orthotics.

These insoles are flat but conform very well if you have a high instep. They initially take up space inside your shoes, but within a few minutes relax so they are more comfortable.

Q: How many millimeter thick are the insoles

I don’t know the millimeters but could not wear any shoe or sneaker that I couldn’t take out the sole because of the thickness.

Q: Some insoles for shoes smell after wearing them. Do these smell?

I haven’t noticed any smell from mine, but I always wear some type of sock with the boots or athletic shoes that I wear.

Q: I wear a 4.5 woman's shoe. Can these be cut down enough without damaging the insoles to fit properly?

I wear a size 10 so I can’t answer your question. I can suggest that if you have a shoe that you can remove the liner inside and use it for a guide to cut them down to size. They are well worth the work.
