The knife seems to fit the hand better when it is held curve down. What makes the curved up grip better?

Sorry, it's not meant to be held like that. the knife was designed to be held with your fingers on the grooves and the blade pointed away from your body. you make a punching motion and deliberately miss your target so that the knife cuts on the way by. Google Lucifer/Maze/Knife to see how it is used in a fight. Maze is a character on the show Lucifer who uses Karambit knives. It's a great knife for self defense.

How large is the finger hole... I don't have teeny fingers?

I have pretty fat fingers and my index finger fits in with plenty of room to spare. You should have no problem. I can’t imagine anyone having a finger too fat to fit. The knife is very sharp too. Great deal.
