I have the 1st edition of the base game, can I play with the 2nd OTB or do would i need to buy the 2nd edition of the base game as well?

No you can't . The 2nd edition cards have a different look to them. If you go to zman games they have a conversion card set. To make the first edition base set compatible with the 2nd edition OTB. and vis versa. It runs I think $20.00 it adds the 2 new role cards that are added to the 2nd edition base set and the new card look.

I understand that five players can play with this expansion set. Are there multiple roles for the fifth player or must you use the bioterrorist?

You can play without the bioterrorist, using five players. "5-Player Game: Pandemic can be played with 5 players. During setup, deal a Role card to the 5th player and 2 Player cards (as with 4 players) to each player."

Why is the picture in Spanish? Is the game in Spanish? I need an English edition.

I ordered the "On the Brink" expansion from Amazon and received the English language version even though the picture shows the Spanish version. I have no idea why the Spanish version is pictured.
