how wide is the sole at the widest point ?

My boot is just shy of 5 inches wide on the Inside at the widest point. It’s 4 inches at the Heel and Gradually gets wider to its widest point at 5 inches max.

What size is a small?

It's hard to say as they only describe it as small. No numbers. My wife is a size 6 and it fit her with room to spare. It has Velcro straps to tighten around the foot ankle area.

Is there a wedge inside to elevate the heal?

There are foam inserts that can be used to elevate the heel. Over time the inserts do wear down.

Is the foot width adjustable?

Yes, you can adjust the width using the straps. They are Velcro so you can make it as tight as you need to. Keep in mind that the bottom plastic part did not seem adjustable. Your foot slides into it, then you pull the straps to tight or loosen boot.
