Sylvania 42511 - FS25/BL/2PK 12CS CP Ballast Starter
One of the Ballast Starters went bad on my Wurlitzer 1015 Juke Box. Like just about everything else I check on Amazon first to see if they have what I need. Sure enough they did so I made the purchase and everything is back to normal on my Wurlitzer Juke Box.
