Q: Has anyone used these pillows on a leather sofa? Do they slip down, or does the trim hold them in place upright?

They are perfect on my leather sofa. They don’t slip.

Q: to replaced the case do you measure the case flat or across the pillow?

Not totally sure of your question but if you buy the 22in pillow covers, your pillow insert your covering should be no smaller than 22in or even better, 24in in size to fill it out good. I would measure from side to side each direction. Hope this helps.
