They are not narrow. They lean towards the wide side.

Would these be too warm for winter school shoes?

Yes, I think they would be too warm to wear indoors all day especially with socks. Unless your child is going to be outdoors a lot or has a school with poor heating (sadly some still are pretty chilly).

My son's feet are 4.5 inches, should I get size 6 anyway? Because it says to order size one up

I ordered these for my son and they were a perfect fit. I don't use really thick socks though because his feet would sweat. True to size worked for me.

What size does an 18 month old wear?

It depends on the child. You should ask the parenta for the size. My son wears a size 7c/8c.
