The needle looks pretty fat, does it say what guage it is?

It's thicker than a sewing needle, maybe twice the diameter but the tip is very sharp. No specifics on gauge.

What do you use the sharp end for?

An acne tool (also known as a blackhead or comedone extractor), is a tool used to remove whitehead and blackhead pimples. Comedone extractors are small tools that usually resemble small rods and have loops attached to either end or contain sharp lancets; they are designed to pull out the core of the pimple without damaging your skin. Before using an acne tool (a comedone extractor), certain preparation is required to prevent further skin blemishes or infection. The sharp lancet is used for piercing the skin to reach infected whiteheads area.

How do you disinfect this? Can you use rubbing Alcohol?

Yes, you can or simply dab a little TCP or any other antibacterial liquid on a tissue, and whisk it over the tips and after cleaning your Lancet, please remember to dry it and store them in a pouch. No point in cleaning your tools and then leaving them lying around. I hope that answers your question
