Why do you picture 2 filters in first picture (those 2 black cups either side of the case) but not included in the other picture. Which one is true?

They're not filters, they're two different lids. One to use for pods and the other one to be used with ground coffee.

Does it heat the water or do I need to pour hot water? Somewhere it said it needs hot water, but that doesn't make sense, where would I heat it

It heats the water. You just need cold water and the coffee pod. Makes brilliant espresso.
