How am I supposed to remove the backing from the adhesive pieces?

Its weird, you remove 1 side and stick it to the new button, then you remove the other side and stick it to the console button. Its basically a thin glu film between 2 peelable covers. Tweezers might make it easier.

Does this still allow it to fit comfortably in the official case??

It will not affect put it in the official case.

Which of the two thickness pads do you prefer and why?

I like the larger ones so each button is found easily, especially when playing fast paced games like COD MW2
