Can you use it if you don’t have a previous tv in your car?

Yes, but installing this (or any other overhead display) is not very straightforward. Two things- You’ll need to be comfortable with cutting a rectangular hole in the upholstery of the ceiling of the car (yep), and be able to run a 12 volt line from the battery to the unit. Unless you have experience with this sort of stuff, get it done at a shop.

Can I connect my indash cd/dvd and play a movie and it will play on the overhead tv also?

I have a pioneer head unit and when I play a dvd on it, it automatically plays on the overhead monitor. May be the way the stereo shop wired it.
