Are you supposed to put this bra on by stepping into it, or by pulling it over your head?

Definitely pull it over your head.......

It says size down what size would you recommend for 38ddd

If you want the cups to be somewhat comfortable, I would stay true to size. But if you don't mind a snug cup, and a more form fitting band, go down a size. For your size ? At least a 2x

250lbs what size would someone recommend if your around this size I am a 44c bust thank you?

I’m a 3XL. I weigh 250 with a 44DDD bust size. The rib band is very comfortable and the cups are Ok too. If you’re looking for comfort, I would do the 3XL.

Can it worn as regular bra under clothes?

I wear my bra under clothes all the time and I very pleased with the look. And it's also very comfortable. Good luck.
