The suckers on the back or recessed into the pillow and I imagine bonded in so no having to sew them back on, they seem very secure and there is a short information slip advising on how to remove them from the surface of the bath without damaging the covering. Very comfy and large enough to support your head and upper shoulders. Much better than the six pound specials you can buy that when blown up deflate in ten minutes! It completes the experience and makes a bath the luxury it should be. The suction cups mean it does not move once attached to the bath, it has a very nice waffle weave type texture to it and is nicely padded so you can lie back and relax. I use to use a rolled up towel to rest my head on while lying back in the bath and this is a definitely the right way forward.
