Everyone should have this!
It's easy to understand and fill out without "lawyerspeak." Two members of our family were in an accident recently and were in a coma. Neither of them had a Prime Directive, both died of their injuries, so it was left up to the family to make all the decisions. It's been a heartbreaking experience. The blow would have been a little softer if their wishes were known to us.
