Serious safety issue - broke on the second cut

I used this blade the same day it arrived. The first cut was fine, but during the second cut a piece of the blade broke off, as shown in the photo. The fragment flew past my ear and could have caused a serious injury. Based on my experience, this is a serious safety issue, and I would not trust this blade again.

I bought this blade for tiling my new shower upgrade. It cut all the tiles like butter. Would recommend. FYI- the tiles were porcelain.
