has anyone used these for a form 4 successfuly?

These forms are approved background check fingerprints card. These are used industry wide in law enforcement and CWP classes. FBI also use the same forms. Yes they are acceptable for form 4.

Does the card has the code on the left top coner, because is very important the code is there

These cards are the exact replica of the US Government FBI cards that all state agencies use. I'm not sure about what code you are referencing, but they are the FD-258 Fingerprint cards. I use them by the 100's every month. i send them to all of the state agencies, and the FBI without a single problem.
