Does the lever have to go all the way up to lift on the down stroke

The lever does not have to go all the way up to lift on the down stroke. You only need partial travel of the handle to lift, this may be obvious to you, but with shorter travel, you will have an increased quantity of strokes needed to reach your desired height. (That was super wordy.)

Where is the jack made? Can you buy a seal kit when you buy the jack?

So I just got off the phone with Arcan and the part number for the seals are RS660258SK and cost $9.20. They also have the power unit assembly which would be the dual pistons and cost $73.05. There phone number is 800.837.5994.

Can anyone confirm the max lift height? Item descrip says 19 1/4" but the "marketing" section lower on the page says 18 1/4"...need 19+.

19 1/4 is the 2 ton. The 18 1/4 is the 3 ton. I have the 2 ton and measured max height from the ground to platform...it measured 20 1/4". So max height on the 2 ton is probably 19 1/4"

Will the low profile style lift a 2012 Chevy Equinox?

I don't see why not. It starts low, and can raise quite high.
