How long is the bar come off the wall onto the main shower head

The upright rail is approximately 75mm (3") from the wall, and the raindrop shower head is approximately 300mm (12") further out from the upright rail. So about 375mm (15") from the wall. The raindrop shower head also can be tilted slightly to allow the water to spray further out from the wall. I hope this helps, mine is great and was very easy to fit to the existing shower.

Can both shower heads be used at the same time ?

Not really, the tap at the base distributes the water through one or the other. Due to the size of the upper head you'd need a lot of water pressure to get a reasonable flow from both at the same time.
