Q: How would you measure 4/64 and 6/64 with this tool? Would it be possible?

It has a spot for measuring 64th's on the bottom left on the front side, but it only goes to 48/64, which should certainly be enough for measuring string height.

Q: Does this item come with instructions, and is it made of steel?

No instructions. It is essentially just a ruler. But the size, shape and multi-denominations make it especially designed for guitar setups. It is made of high-quality (stainless?) steel with black etchings.

Q: I tend to use 32nds for setting up string heights on my bass. How do you use that area when it's in the middle of the rule?

You use the 64ths. two 64ths = one 32nd. Either that or saw a hole through your bass and stick the tool through
