What are the exact dimensions

There is a pdf CAD drawing with dimensions available under the specification tab in the amazon ad. OD of the cover is 14.18"; center of screw to opposing screw is 12.68"; thickness is 1.60"; diameter of the inner lip which fits inside the actual pipe is 11.35"

How to take the lid off? I only need the vent part to keep debris out of culvert.

There are spots for Panhead screws to attach it, however it just slides into the pipe until it hits the lid. You should be able to remove, but it was quite a snug fit for the corrugated pipe I had.

Can this lid be used for a concrete septic tank?

I did, I used it with a riser though
