Yes you grip/squeeze the handle to hold t he paper.

Q: Is there a bend in any of these 12, 15 or 18 inch

I have the 18" there is no bend, at least not easily. I guess you could use a vice or similar) They have a curve. Works better than any of the others I have tried.

Q: It says the product dimensions are 3x3x25 but then you stated it was 4.5” wide at the handle, I need case what are exact dimensions?

15 inches long 4.5 inches wide at the handle ,I just measured it.

Q: can this also be used to lift and hold your testicles out of the toilet water during defecation?

I'm female so technically I have no experience here, but I don't see why it couldn't. It has rubber on the end so it's soft. It seems long enough. I would worry about the pressure but you control it with your had. It might work how you need it to work without using the tongs part. Hope that helps.
