Q: can the cleat be left on? or does it have to be removed?

You have to unscrew the screws holding the cleat on, then you reattach the cleat using the new (longer) screw with a spacer. But your cleat will remain.

Q: Will these fit a 2023 F150?

purchased set for Jeep Gladiator and the mounting tabs were way off and would not fit without major modification ended up sending them back so take that for what it's worth
