Yes this product is unisex and can be worn by women or men.

Q: Dose the thickness of the support pads make a difference?

The pads work great. Keep hernia from coming out. The pads are the correct thickness.

Q: Can this product be underneath undergarment versus over? I notice in all the pictures it is over the undergarment.

I think the strap would cause chafing without the undergarment to soften the strap's contact with genitals. I also find it useful to pull up and fold the top elastic band of my briefs out over the top edge of the truss belt to prevent the briefs from creeping down beneath the truss belt and wrinkling, thereby causing chafing and itching.
