Can this also be used to wash the back of your neck as well?

You certainly could use it to wash the back of your neck. But, if you purchase it for that purpose, use the side with the larger nubs. One side has almost brush-like fibers that might scratch tender skin. I like to use the nubs to spread lotion on my back, so they would be a gentler washing surface as well.

Is this antimicrobial?

Save your money, buy a good bath brush. This is a worthless and wasteful way to spend money

How often to replace?

I purchased, love it. I have no idea how often it would have to be replaced. Can’t imagine it would have to be replaced very often, if at all!
