Does it have to be stained to look good or good as is?

Staining is purely an option but I would use a varnish or urethane product to protect the surface from future contamination.

Does this come with the correct adhesive or do I need to purchase that separately?

You will need to purchase contact adhesive and apply per the instructions.

Can it be painted instead of stained?

Yes of course, I used it to repair a painted cabinet door. That being said the grain of oak is fairly discernible even after sanding and painting (which is fine if you are matching painted oak). If you expect a smooth finish I suggest selecting a birch product which has much less graining.

Should the paper back be removed prior to applying adhesive?

No absolutely not..the backing is there too provide stability and integrity to the thin layer of wood. Apply the adhesive directly to the backing. Also suggest watching a few online videos on the subject…..contact cement DOES NOT allow you to adjust your work and requires precision.
