Are you supposed to take out the existing sole of your shoe?

Place it on the existing sole. I was not pleased with the insole as 1/2 inch was not enough to make a difference. I reordered a 1 inch much better.

does each insole work in either the left or right shoe?

No. They are specific to right and left shoes. See my review for a picture.
