It does have water bottle pockets, but they are kind of tight.

Q: Does the waist strap come off or is it on there for good?

The waist strap is easily removable. No need to cut off as the other reviewer mentioned. I keep mine off normally, but when I take it running, I put it back on to keep the bag from bouncing too much.

Q: Can you put a water bladder in it and run the straw through the top hole on pack?

yes, if you want to modify or create you own hole.. they is no top hole to run no straw, but beside that is a great pack , I had for over 4 years ,,
