Can one arm be either left off when assembling this or can one arm be folded down?

Yes, but you can also use the arms different on either side (wide vs narrow holes) and that's what I did to get around the tiolet paper holder on one side. Even then it's still easy to sit on and wide enough for my big behind. My mother in law (who we bought it for) doesn't mind one wide and one narrow side.

does it have to bolt to the toilet

The rail bolts to the toilet base, on both sides

Using this rail on both of my toilets caused the gasket or wax rings to start leaking. Has anyone else had this problem?

I have not had any leaking with the Guardian safety rails ' Since the wax seals are at floor level and the toilet is secured with bolts I'm assuming that they need tightening .If the bolts were loose to start with then applying torque with the rails could cause the seal to break like using a giant wrench .

Yes it does fit elongated toilets and it's sturdy. It's attached with the same bolts as the toilet seat.
