Does it come assembled?

No, you have to assemble it using the spanner, hex key and screwdriver included in the pack. The assembly instructions are excellent and each part is numbered so it is very difficult to make a mistake when assembling.

Unfortunately no, it is just designed for walking, I have tried to run on it but you cannot control how fast you go and you end up falling off, plus it makes so much noise when trying to run.

Can I put it on top of carpet?

I have put it on carpet as well as well as laminate with no problems.

Does the belt slip to the side like other cheap treadmills?

Maybe it's just the way I walk but yes, the belt immediately starts slipping to the left when I use this. I have applied the lubricant as instructed and everything is set up, but within 5 mins the belt will be entirely to the left of the track, which is frustrating as I have to keep on re-aligning it.
