I had a friend order these for me and they came with japanese writing on the cap. there was no "pentel" branding as in pics. did i receive a fake?

The same with my pens and my friend as well. No, I don't think they are fakes. And, I believe on the order site, it tends to let you know that these pens are made in Japan. I have ordered other pens from Japanese manufacturers, and all are very good.

Do you receive one of each color: black, blue and red?

Yes, and none of the colors run or smudge..

does it smudge if you’re left handed?

I am not left handed but this pen does smudge easily so I would guess that it would.

Can the cartridges be refilled using a syringe and bottled ink?

Since it is a cartridge replacement, it cannot be replenished with bottle ink.
