It works really, really well. If you want poor headset recordings, you can go cheaper though. There is never any distortion with this cable.

Is there a way to have a combination of through-the-cable audio and audio of engine noise, etc? It would be nice to hear some engine sound.

As others have said, this will only record the same audio you hear in your headset. That said, I use a cheap sound recorder to get ambient and engine noise. Then splice in during post.

Can you plug this in without a headset to the plane’s Audio received jack? Will it hear cockpit plus ATC sound without headset in between?

Yes. Audio will be passed through without a headset attached. In my plane I use this without a headset straight to my go pro.
