Do they have stick on backs?

No, they are hollow and have small openings so the earring back is covered on both sides. At first I thought the openings were too small but they can be maneuvered to get them on. I haven’t worn any with these yet but I’m guessing I will now be able to wear my earrings again without my ears hurting after an hour. Hope this helps you.

How can these help when only fit over clip part of earring and other side of ear remains "uncushioned"?

It really does make a huge difference. Most of the clip-ons I have seen have a fairly smooth backing that fits on the front of the earlobe. The back part is usually the one that needs cushioning. However, if you need some cushion on the front too, you can slice one of the Maxdots in half and glue it to the earring.
