How does this glue handle high heat? 150 F plus (near hot equipment)?

I can’t say that it will fail but it will soften. It is a PVA type glue and that becomes as soft as rubber cement when heat is applied. I would recommend a polyurethane, epoxy, or resin glues near a heat source to avoid damaging the glue bond via heat.

Should I apply glue to substrate and the veneer or only one side?

I apply the Unibond to the substrate only. Press hard with a veneer hammer if you have one and clamp well or use a vacuum bag. I have applied heavy glue once and the veneer wrinkled. Good luck.

Will this work on exterior wood trim? Will this work on pvc trim?

No this item is not good for exterior trim. I would suggest the Titebond III for exterior wood trim and for the PVC trim I would suggest the DAP Dynagrip product.
