this monitor works to play in a ps4 console to play nba 2k or mlb?????

No this is not a 2k monitor it's a 4k monitor. So it will only play mlb and maybe madden. Def not 2k.

What is the refresh rate?

Mine overclocks to 74hz. That's a 23% frame rate increase over the stock setting, and it makes a huge difference.

What cords are included (Power supply, HDMI, VGA, DVI????)

Mine came with the power cord and D-sub cable. Other cords would have to be bought separately, but luckily could probably be found for pretty cheap (such as the HDMI cable I ordered yesterday: http://www.amazon.com/gp/product/B003L1ZYYM/ref=oh_aui_detailpage_o00_s00?ie=UTF8&psc=1).

Is the screen glossy or matte?

The screen is matte. Best contrast ratio I have seen in a monitor. Best gaming monitor I have owned.
