How large of neck will it fit

Hello Tj,This cervical neck traction device can stretch a neck up to 7,5 inches in height, and wlll fit around 5 inches in diameter. There are fully adjustable velcro straps that you can use to adjust the traction device, and to fit it nicely around your neck.Good luck!

My pinched nerve pain only occurs at night. Could I sleep with this on?

We do not recommend sleeping with the neck stretcher on, because if you lay down with it, it actually stops stretching your neck - your body needs to be vertically positioned and the head must not be laid down in order for this device to work. We do recommend using the device a few times a day, and right before you go to sleep, you will definitely feel the difference after a few days. Please be persistent and use it at least 4-5 days without missing a session, because your body and your muscles need time to respond. We hope you sleep better by using our device!

I do 25 pounds of pressure at PT would this be about the same?

no I'm no expert but 25 pounds of pressure seems way more. I do get some relief from wearing this. I get "knots" or "bulges" on the sides of my neck and this will help some with that pain. Sometimes I think some sort of weighted pulley would be better. hope that helps
