Does the adhesive need to go on a hard surface (plastic) or would it be ok with leather? Would the adhesive ruin the leather when pulled up?

I've used it on the leather and plastic dash. No sticky residue left behind.

Does this product come in a tape or roll form, instead of rectangles or pads?

Very sticky by itself ,so no need for tape

will this damage the dashboard if i want to remove it. will this work for a dash heater.

It's so sticky and soft that I think it will. It doesn't keep its shape. I didn't want to use it because I suspected it'd leave marks on the dash if removed. throwed it away. If it's in hot sun light, I think it'll melt into the dash.
