No, it does not have a knife inside it. But it has a significant puncture point on the top of the pen.

How do you retract the pen so that it doesnt write all over your clothes?

You have to twist Tue pen in the middle about 6 tim es and the cart rage will retract if you twist about 15 times that's how you change the cartrage
