Yes, I put it on a 1997 379 Peterbilt also.

The one I received was 24 volt and still works.

Will this fit a 2004 peterbilt 387?

It won’t fit because it’s kinda small but what I did on my own was to compromise by drilling smaller holes to fit in mine. The original one it’s about $350-$500
