Its about 4 inches across, the trinket dish is much smaller

The box says nothing about any lead content, so I would assume so. Usually leaded crystal has a percentage lead figure on the box. The little tag inside also makes no mention of lead.

can put some color sand in it?

Sure. The bottom or bowl will hold maybe 1.5 cups of sand or whatever.
