I have played rugby for 12 years - this is the ONLY mouthguard I like to use. I've used this exact style for 10 years. It molds well - has enough padding for absorbing contact - and most importantly I can breathe and talk while wearing it. I forget I have it in. When they stopped selling them in local sports stores and only sold bulkier Shock Doctor mouthguards, I bought 5 of these on Amazon. Can't recommend this mouthguard strongly enough...

does it come with a top and bottom, or do you have to buy it separately?

The mouthguard only comes with one piece. This is a "single" mouthguard. If you need one protects both top and bottom teeth, choose a "double" mouthguard.

Is this a boil and bite mouth guard to conform to your teeth?

It fits much better if you do boil & bite. It's a great mouth guard. Nice protection.
