Do the beads stay put? Or slide to the sides of the blanket?

Just received my blanket yesterday and already the beads slide to the outer edges. The little "pockets" obviously have little spaces where the beads move through and the entire purpose of having a weighted blanket is defeated. Which is really too bad, the rest of the overall quality of this blanket is actually decent for the price. I will be returning this and opting for a higher quality blanket.

Does it come with the duvet or not? Some comments say “this duvet,” as though this is the duvet, although it says it’s the weighted blanket.

No, it's just the weighted blanket. I bought a cover for it separately- from a different weighted blanket company actually.

Did the machine wash not hurt it? What temperature should I use?

I recommend to put it in a washing machine that can handle that weight; but to answer your question, no the washing machine does not affect it at all. I used warm water and washed it normal. For drying it was a normal dry as well.

The glass beads are moving around within the quilted squares. Is this supposed to happen?
