It will not hold a baby's toe nail in place. It is too large. It is meant for an adult. If you are referring to a pinky toe nail the answer is ....maybe. You would have to position the protector over the nail. The foot activity should also not be aggressive and could offer protection instead of a band aid.

How well does it stay in place during an 8 hour work day walking the entire time?

I wear the pinky toe sleeves all day and into night until I go to bed and it stays on the entire time. Never had a problem with it coming off. You just make sure you place it on correctly and it is flat and when you put your socks on just make sure it is smooth underneath.
