My child is 41" tall. Toddler 2-4 sizesays 35"-39". The next size (sm) says 44"-48". Any advice from owners? I don't want it so large it falls off.

We purchased the Toddler 2-4 because our little one wears a size 4T. It would have been too short for her without leggings / tights. My recommendation would be to go with the larger size. You'll be able to tighten it at the waist with the belt, and hopefully your little one will be able to wear it for longer than a week! They grow so fast!
