Im confused , its for the ear but says external use only.

It's perfectly safe to use in ears (human or animal). By "external use only" they just mean don't swallow it. Good product. I mainly use it in my dogs ears to suppress yeast and help prevent ear infections. I bought a second bottle for my own personal use.

Why does the picture say Pancrease oil?

I'm not sure.. but my bottle says garlic oil on it.

Can I use this for my hair ?

Well, there is no reason you couldn't, it is pretty healthy stuff, but it would make your hair very greasy.
