Q: How thick are these inserts?

They are not to be compared to a cushion insert. They are a simple flat, thin carbon insert.

Q: Which side is supposed to be "up" (ie against your feet)? The side with the numbers or the other? Or does it matter?

The top side goes up! Sorry couldn't resist. I always put the number side up and they worked fine.

Q: What is the most eco-friendly way to dispose of these insoles?

Right off top of my head I would say shred them very fine and use them in gardening. The soles ate activated charcoal and sponge. Put the shreds where you want them to help stop weeds.
